Baylor Expert: Don't Lose Focus on Spiritual and Mental Well-being

Baylor Social Work professor shares tips and resources during stressful half-way mark of semester  Holly Oxhandler, Ph.D., LMSW., associate dean for research and faculty development and associate professor in the Diana R. Garland School of Social Work.  October marks the halfway point to the fall semester and can carry all the excitement, stress and anxiety of another academic term winding to a close. The month also typically marks one of the busiest times of the year for university counseling centers across the nation. With unique challenges in 2020 related to COVID-19, the University has acknowledged those hardships for all in the Baylor Family by taking the initiative to focus on mental health throughout October. Baylor University’s Holly Oxhandler, Ph.D., LMSW, associate dean for research and faculty development and associate professor in the Diana R. Garland School of Social Work, is an expert on mental health, primarily anxiety and depression, as well as religion and spirituality in clinical practice. In this Q&A, she shares tips and resources to students, faculty and staff who are facing all of the typical challenges of another mid-term while also navigating a global health crisis. Q: The 2020 fall semester provided challenges and obstacles never seen before in higher education. Why has COVID-19 been a uniquely stressful influence on the mental and spiritual well-being of students, faculty and staff? There are a number of reasons COVID-19 has been a uniquely stressful influence on the mental and spiritual well-being of those in higher education. First, there are the unanticipated layers and learning curves of new considerations and adjustments to course delivery, safety protocols, dining, campus activities and communication, to name a few, on top of the typical adjustments and emotions we all navigate at the beginning of each fall semester. Second, those in higher education entered fall 2020 without the “typical” summer that’s needed to reset, make adjustments and plan for the upcoming year’s activities. For example, many administrators, faculty and staff spent the summer not only planning for the usual upcoming academic year but also reconfiguring classes to be hybrid or online or simultaneously include both in-person and online students, in addition to modifying student activities and faculty research project timelines and plans. For degree programs that include internships, like social work, there have also been additional layers of consideration in order to comply with our professional accreditation standards. Third, we recognize that many students, their family members, as well as Baylor faculty and staff members’ loved ones, have been affected by the economic impact of COVID-19, adding a layer of financial stress. Fourth, many within the Baylor community – staff, faculty and students – have needed to juggle childcare and homeschooling their children as they continue to engage in their own work and/or educational expectations. Finally, the fear of contracting COVID-19, especially among the most vulnerable and high-risk populations, has been constantly present. Not only are we adjusting to this academic year with new ways of being and new protocols that keep us and one another safe, we’re also facing unexpected waves of fear and layers of grief for the missed events, opportunities and connections we had hoped to experience. As resilient as our community is, I think it’s important to remember we cannot “operate as usual” because things aren’t usual. Instead, we must allow ourselves and one another the time, margin, flexibility and grace needed to sit with and move through the rising emotions, grief, stress, loneliness, fear and uncertainty as they come. We cannot skip or bypass these emotions, but instead, must move through them, often with the support of loved ones and/or a trained mental health care provider. Q: Mental and spiritual health are challenging during even a typical semester experience. What are some of the effects and reactions you’ve witnessed to these circumstances among students, faculty and staff compared to a non-COVID-19 semester? As human beings navigating a global pandemic to the best of our ability, our mental and spiritual health have all been impacted to some degree this semester as we have individually and collectively faced a number of unexpected difficulties. I have also seen a beautiful response to the reality of this collective struggle in my interactions with Baylor faculty, staff and students that includes deep empathy for one another and an increase in valuing authenticity as we engage in the high-quality, meaningful work we each do. It has been a gift to witness Baylor community members holding space for colleagues’ and students’ vulnerability as we admit this is hard for various reasons and recognize that we cannot just push our way through this season. When we admit this isn’t easy and that we are all juggling so much to the best of our ability through thick layers of uncertainty, it gives those around us permission to admit their experiences, too. In fact, I think when we create space for that shared vulnerability and empathy in our interactions with others, we can better assess the current situation, remain present to one another and discern what steps are needed to move forward together, particularly because we’re not carrying an additional layer of effort pretending that everything is fine. That said, the Garland School of Social Work conducted a couple of well-being surveys since this summer to internally check in on how our faculty and staff are coping with this season and identify the biggest stressors they’re facing and sources of support. Our faculty and staff have also been continually checking in on our students through this season. Not only do we see many noting the same stressors that we’re all facing these days, normalizing how difficult this is for each of us, but the act of nonjudgmentally holding that space for ourselves and one another has been a tangible step of offering the care we know is uniquely woven into the Baylor experience. Q: How can individuals within the campus community tend to their spiritual health to close out the semester? I would invite readers to take a moment to pause and identify a few spiritual practices that uniquely support them well, even if that means thinking back to less stressful seasons. The key to note here is that these are practices which require regular engagement, similar to if we were to practice a new instrument or sport. Spiritual practices can vary based on our faith tradition and may include praying; meditating; centering prayer; reading our religious text; walking a labyrinth; journaling; practicing gratitude; listening to a sermon or faith-based podcast; praying over and contemplating scripture; engaging in creativity; practicing daily examen; or listening to spiritual music. Some practices may involve other individuals that can be done safely, including seeking spiritual direction, participating in a Bible or faith-based book study with others or engaging in worship (even virtually!). These practices can offer a sense of groundedness and a reminder that God is with us, including through this season. As we continue to navigate this season of uncertainty, it is critical that we intentionally weave in spiritual practices that offer rhythms, routines and a grounded faith that can support us well through the waves of difficulty. Especially on campus, I would encourage Baylor community members to follow along with Spiritual Life’s resources and events, or check out Better Together BU, a partnership supported by both Spiritual Life and Multicultural Affairs. Q: How can individuals within the campus community tend to their mental health to close out the semester? Tending to our mental health in this last stretch of the semester will be so important as we move into the stress of finals, the complex emotions tied to the holidays, grief with upcoming celebrations looking different this year (including how we celebrate holidays, who we celebrate with and the reality of many having lost loved ones to COVID-19) and the reality of seasonal affective disorder on the horizon. In fact, in a typical year, about 5% of U.S. adults have seasonal affective disorder (SAD), with another 10-20% having mild forms of it. In light of all of the added transition, uncertainty, complexity of caregiving and homeschooling while working/studying at home, layers of loneliness and grief, I do hope our Baylor students, staff and faculty will actively prioritize taking good care of their mental health and supporting others’ mental health care, too. One way I highlight this with my social work students is by recommending creating a self-care plan that pays attention to our physical health, mental health, social support and spiritual health. If we can identify some strategies to holistically care for ourselves well and be mindful of potential barriers to navigate, we may have more resilience and practices to draw from to cope with challenges and stressors that arise. Finally, although NAMI highlights that 1 in 5 of us are currently facing a mental health struggle, some studies have shown that over 80% of us will meet criteria for a mental illness by young adulthood or middle-age. Therefore, I highly recommend that anyone who is noticing any changes in their mood, diet, sleep habits, behaviors or overall well-being immediately reach out for help. Students are encouraged to reach out to Baylor’s Counseling Center, CARE team or the BARC. Faculty and staff also have resources available through Baylor’s employee assistance program. Other resources for finding a mental health provider include HelpPRO, Psychology Today, Low Cost Help or these additional resources. For those who are deeply struggling, please reach out to the Suicide Prevention Lifeline at 1-800-273-8255 or text ‘HOME’ to 741-741 for the Crisis Text Line. As part of my faith and my social work values, I believe that each of us are worth caring for ourselves, including caring for our mental health alongside our spiritual and physical health. Q: What successes or bright spots have you seen within your campus experience that offer encouragement to how the Baylor Family has handled the crisis throughout the semester? I am regularly amazed by the Baylor students, faculty and staff, the ways we have navigated the crisis together this semester, and I am especially grateful for President Livingstone’s and Provost Brickhouse’s leadership since March. This semester, some bright spots have included Dr. Deborah Birx’s reflections on Baylor’s efforts to keep everyone safe from COVID-19, the Fall Faculty meeting and Dr. Peter Hotez’s appreciation of how Baylor leaders have kept the Baylor and Waco community safe and following along when Baylor students take over Baylor’s Instagram account (like Brandon Nottingham’s takeover on World Mental Health Day!). As the Garland School of Social Work’s associate dean for research and faculty development, I have also loved learning about the ways so many Baylor faculty are offering their unique research expertise and wisdom to serve others through this difficult time, such as Dr. Emily Smith’s “Friendly Neighbor Epidemiologist” Facebook page to explain COVID-19 information. I’ve also been reminded of what a gift it is to be a part of the Garland School of Social Work (GSSW) and this community of faculty, staff and students. The resilience, creativity, love for serving others, dedication to the social work profession and care for our students is so apparent within the GSSW. I have especially seen how my faculty and staff colleagues have adapted courses and assignments, creatively considered students’ needs and juggled their research responsibilities while extending grace to themselves and one another as we navigate this season together as a school to the best of our ability. Similarly, seeing our students’ resilience, flexibility, support of one another, commitment to the profession and heart for the clients and communities they serve is truly inspiring. Finally, Dean Jon Singletary’s servant-leader heart for the GSSW and the ways he has supported our school through so much transition over the last five years has been a gift. One example of this includes the two hours of weekly well-being time he extends for all GSSW staff and faculty to use in support of our spiritual and mental health care. Q: What gives you hope for the spring semester and beyond as students continue through their academic endeavors? Truthfully, our students’ presence and their enthusiasm over the fields of study they are dedicating their lives to gives me hope. As a professor, there is nothing like watching a student become fully alive in the work they are passionate about and feel as though they were made to do. Our students’ willingness to fully participate in the transformational education that Baylor offers, especially in this difficult season of COVID-19, is an honor to witness as a professor and certainly gives me hope. Further, seeing the ways our students are empathically caring for their neighbor by following Baylor’s safety guidelines, growing in their faith, checking in on one another, understanding faculty and staff are doing their very best and continuing to demonstrate their determination to learn and grow is an inspiration. My hope and prayer for our students as well as our staff and faculty colleagues as we move through the remainder of the fall semester and into the spring is that they rest as they need to and prioritize taking good care of their mental and spiritual health. I also pray that we recognize as a community that by caring for our spiritual and mental health, by taking this season one day at a time, by trusting we are doing our best and by reaching out for help when needed, we give others permission to do the same.

Don’t Neglect Spiritual, Mental Health During this Time of Health Crisis, Baylor Expert Says

In a difficult and ever-changing time of crisis surrounding the spread of coronavirus, the basic needs of health and safety come first. But as these basic physiological needs are met, the more advanced care for spiritual and mental health can remain overlooked or ignored altogether. Baylor University’s Holly Oxhandler, Ph.D., LMSW., associate dean for research and faculty development and assistant professor the Diana R. Garland School of Social Work, is an expert on mental health, primarily anxiety and depression, as well as religion and spirituality in clinical practice. In this Q&A, she shares tips and resources to help unhook from the baser instincts of fear and anxiety, even momentarily, to monitor and care for spiritual and mental health needs. Q: During a crisis, why do spiritual and mental health needs tend to be overlooked or ignored? A: In the midst of a crisis, our natural reaction is to go into a fight-or-flight response to the situation. Our most basic needs must be met, such as finding a sense of safety, and our bodies are doing what they were designed to do: to protect us. For example, if we were to encounter a bear on a hiking trail, our sympathetic nervous system would be activated to meet the basic need of keeping us safe. Our spiritual and mental health are not primary needs in that moment of crisis. If we were to run from that bear and reach safety, our emotions would eventually become regulated, our breathing would become normal, and we could return to a generally balanced way of being that allows for attention to our spiritual and mental health. However, we are in a prolonged moment of crisis surrounded by uncertainty, constantly evolving news updates and daily threats to our and loved ones’ safety regarding our health, finances and sense of normalcy. In this state of constant stress, it can be really hard to unhook from the fight-or-flight response and remember to tend to our spiritual and mental health. Q: How can people tend to their spiritual health during this time of crisis? A: In this moment, most of us are being forced to be still and/or surrender the illusion of control in ways we have never faced before. In this stillness, our spiritual practices can help remind us of a divine Love that is with us through each moment, but we must intentionally set time aside to practice them. Plus, many research studies have shown healthy, positive spiritual practices have the potential to support our mental and physical health. One thing that’s very important, especially in the midst of this crisis, is that we do not spiritually bypass what’s happening. It may be tempting to want to jump to hope and ignore the pain, but to the best of our ability, our faith traditions teach us we must sit with and feel the grief rooted in the overwhelming change and loss we and our neighbors are facing. As Fr. Richard Rohr says, “If we do not transform the pain, we will most assuredly transmit it – usually to those closest to us.” We must be with the fear and uncertainty, grieve the loss of life as we knew it a few weeks ago, pray the psalms of lament, and feel the freedom to wrestle with and/or cry out to God in ways we read about others doing so in our sacred texts. The important thing is that each of us engages in something tied to our faith, regardless of what we believe in, and to be consistent in the practice, continually learning to surrender that sense of control we’re all finding ourselves learning to do right now. Spiritual Health Tips Prayer Reading our sacred text Meditation Centering prayer (my personal favorite, which teaches us silence, solitude and stillness) Breath prayers (here are some examples) Engaging in creativity (music, dance, art) as an act of worship Practicing gratitude Seeking beauty in the mundane Yoga Journaling Especially in this time of increased isolation, I would recommend inviting others into these practices for a sense of solidarity and community, including those within our home or via social media, video conferencing or by phone. Q: How can people tend to their mental health needs? A: The first practice I would recommend is to pause and breathe deep for 4-5 seconds, noticing your belly rise instead of your chest, and breathe out for 6-7 seconds. The second would be grounding. When our brain is flooded with information and emotion, it is hard to stay present. In grounding, we take a deep breath and ask ourselves five questions to return to the present moment: What do I smell right now? What do I taste? What do I see in front of me? What can I touch and feel the surface of? What do I hear? A third recommendation would be to get outside as often as you can and, ideally, into some sunlight. I would also recommend the practice of tuning into our bodies. For many of us, it can be difficult to pay attention to what our bodies are trying to tell us, in the same way our thoughts and emotions communicate with us. Especially now, pay attention to the tension in your neck, the overall exhaustion, the pain in your arm muscles, the tightness in your chest, or any other experiences you notice by tuning in. Your body may be trying to tell you to spend less time watching the news (maybe cut back to 1-2 times a day), to take a nap or to move it and exercise. Last, and perhaps most importantly, do not hesitate to reach out for help if you are noticing changes in your appetite, sleep, mood, thoughts or feelings. Stay in communication with loved ones but just as you would seek a medical doctor for a broken arm, seek a therapist when you notice changes in your mental health. Many therapists are quickly adapting to telehealth services in this moment to meet growing demands and social distancing expectations. Some sites for finding a therapist include Psychology Today or HelpPRO, and if you or a loved one are deeply struggling, please reach out to the National Suicide Hotline or call 800-273-TALK (8255). Reaching out for help with your mental health is a sign of courage, not weakness. Q: During this time of crisis, what populations do you feel are most vulnerable to mental or spiritual health decline? A: Honestly, I think every single one of us are vulnerable to mental and spiritual health decline during this moment for varying reasons. As mentioned before, these parts of us are easily brushed aside when we’re most focused on ensuring our basic needs of safety and security are met. Still, there are a few groups I’m especially sensitive to. The first are the helpers on the front line facing far more need than resources, time or energy to meet those needs. These include our health care providers, social workers, therapists, grocery and restaurant employees, teachers, parents, nonprofit organizations, faith leaders, volunteers, community leaders, pharmacists, lab technicians, scientists and more. These helpers are at such high risk of burnout and my hope is that, to the best of their ability, they are caring for themselves holistically in order to care for others well. I’m also sensitive to a few other groups who are vulnerable to mental and spiritual health decline. First, older adults are the most religious cohort and are already at risk for depression and isolation even without a crisis, so I worry for them in light of the necessary social distancing. Second, in light of added pressures, those with various mental illnesses must continue to care for themselves via the medication or therapy they need. Third, I’m sensitive to those who are recovering from a substance or behavioral addiction and are now surrounded by overwhelming stressors that put them at a higher risk of relapse. Thankfully, recovery groups are now moving online and I would encourage those in recovery or remain plugged into a group. Q: Many people are unable to access a doctor or professional therapist to get help. What are some resources for those that may need free or low-cost options? A: There are a number of websites that can help individuals find a professional mental health care provider, such as a licensed clinical social worker, psychologist, counselor or marriage and family therapist. I mentioned Psychology Today, HelpPRO, and the Suicide Prevention Hotline above, but there are others. Better Help and Talk Space are two growing online therapy sites and Low Cost Help elevates providers with affordable rates. I also host a weekly podcast, CXMH: Christianity and Mental Health, which has a ton of episodes on various topics related to this intersection, including a recent one specifically on COVID-19. You can also contact your insurance provider to see who is in network or, if you do have a primary care provider, see if they have any recommendations. Your local faith communities may also have a list of mental health care providers to consider. There are also many organizations committed to providing resources, including the Substance Abuse & Mental Health Services Administration, National Alliance on Mental Illness, and the National Institute on Mental Health. Q: How can neighbors help neighbors and individuals help individuals outside of the professional or therapy setting? A: I really want to emphasize that, especially in this moment, we must practice extending grace, being still and holistically caring for ourselves so that we can care for others well. We can do our part to serve those who are serving others, even if that means practicing social distancing and stillness, while tending to our spiritual and mental health. Discerning what is ours to do in this moment to offer help, healing, hope and love to our neighbors as well as ourselves, and to help protect our helpers from burning out, will be critical in the days and weeks ahead. As Chris and Phileena Heuertz shared in their prayer, A Call to Solidarity During COVID-19: “You’re not alone. We’re in this together. Baylor School of Social Work Awarded Grant to Assess Religion and Spirituality in Social Work Education

The Spencer Foundation recently awarded Baylor University’s Diana R. Garland School of Social Work a $50,000 grant to study if and how accredited Master of Social Work (M.S.W.) programs incorporate curricula around religion and spirituality (RS) into social work education as well as assessing faculty views around the topic and how universities’ religious affiliations impact M.S.W. education. The School of Social Work’s Edward C. Polson, Ph.D., assistant professor, and Holly Oxhandler, Ph.D., assistant professor and associate dean for research and faculty development, are co-principal investigators for the study, “Graduate Social Work Faculty Views on Preparing Students to Ethically Integrate Clients’ Religion/Spirituality in Practice: A National Survey.” As of spring 2019, there were 290 M.S.W. programs accredited by the Council on Social Work Education (CSWE), but little is known about how or if RS content is delivered within these colleges and universities. “Given that social workers provide the largest proportion of mental healthcare services in the U.S. [according to the Substance Abuse and Mental Health Administration], and clients have expressed a preference for integrating their RS in previous research, this is especially important to consider,” Oxhandler said. “In addition, no study has examined the potential impact religiously-affiliated colleges or universities have on M.S.W. education, including preparing students to ethically integrate clients' RS.” The research team, which will include a graduate assistant from the School of Social Work, will survey the approximately 5,000 full-time faculty across all of the CSWE-accredited M.S.W. programs to answer three questions: 1) How do M.S.W. programs prepare students to ethically integrate clients' RS in practice (if at all)? 2) What are M.S.W. faculty members' views and behaviors related to educating students on RS in social work? 3) In what ways, if any, do colleges' /universities' religious affiliation impact training on CSWE competencies in M.S.W. programs? Over the last few decades, research has shown a growing awareness that RS plays an important role in Americans' lives and the ethical integration of clients' RS in mental health treatment often contributes to positive outcomes. A previous study by Oxhandler revealed social workers held overwhelmingly positive attitudes toward integrating RS into treatment, but few were engaged in the practice. The study indicated only two practitioner characteristics that could predict their orientation toward integrating clients' RS into treatment: intrinsic religiosity (degree to which practitioners are internally motivated by RS) and prior training. This lack of training was reflected through surveys showing only 13 percent of social workers had taken a course in their M.S.W. program. Further, in another study by Oxhandler and Baylor alumna, Kelsey Moffatt, the team found a mere 78 of 257 programs mentioned a course on RS on their school websites in 2018. “We look forward to learning more about how M.S.W. faculty across the US integrate religion and spirituality content into their M.S.W. curricula as an element of clients’ culture and coping skills,” Oxhandler said. “Recognizing that clients tend to prefer their therapist assess and discuss the client’s spirituality as it relates to treatment, coupled with the fact that very few mental health care providers are trained in this area, we saw a need to better understand what's happening within M.S.W. programs.” The Spencer Foundation has been funding education research since 1971 and is the only national foundation focused exclusively on supporting education research. Biography

Holly K. Oxhandler joined Baylor University’s Garland School of Social Work in Fall 2014, upon completing her Ph.D. at the University of Houston. Her research focuses on the intersection between ethical and effective integration of clients’ religion/spirituality and the evidence-based practice process in mental and behavioral health treatment. She developed the Religious/Spiritually Integrated Practice Assessment Scale, which assesses mental healthcare providers’ (social workers, psychologists, counselors, marriage and family therapists, and nurses) attitudes, perceived feasibility, self-efficacy, behaviors, and overall orientation toward integrating clients’ religion/spirituality in practice.

Dr. Oxhandler has clinical and research experience working with older adults with anxiety and depression at the Baylor College of Medicine, which included co-authoring a cognitive behavioral treatment manual and counselor workbook that integrates clients’ religious/spiritual beliefs into practice. Though the integration of clients’ religion/spirituality in mental health treatment is her primary area of scholarly interest, her scholarship has extended to or intersects with evidence-based practice, serious mental illness, social anxiety disorder, virtual reality, mentoring, and social work practitioners’ professional identity.

Dr. Oxhandler’s research and training have been generously supported by the John Templeton Foundation, the Gulen Institute, and the Fernando J. Zuniga y Rivero Foundation. She is grateful for these sources of support, but is especially thankful for the invaluable mentoring she has received over the years. Because of her exceptional mentors, Dr. Oxhandler deeply enjoys paying it forward by mentoring students (BSW, MSW, and PhD) outside of the classroom with regard to professional development.

She is married to Cory Oxhandler, and the couple is both proud of and grateful for their two children, Callie (2012) and Oliver (2016).

Young adulthood (18–25 years old) is a period in which the onset of mental illnesses peaks. For young adults with serious mental illness and histories of adversity, access to appropriate, culturally sensitive care is critical. Religion and spirituality (RS) are interwoven into many individuals’ culture and are increasingly recognized as important constructs worth considering in the assessment and treatment of mental illness. This study examined data from a qualitative study of 55 young adults with serious mental illness who had used crisis emergency services to explore (a) how vulnerable young adults in psychiatric crisis talk about RS and (b) how religion/spirituality emerge in the narratives of their experiences, understanding and management of their mental health problems. Thirty-four of the 55 youth described RS organically within their interview. Across these interviews, four themes emerged: positive RS coping, negative RS coping, relationship with God, and implications for RS and mental health. Further, RS was described as a very complex topic for this sample, suggesting training is necessary for mental health care providers to appropriately assess and integrate this area of young adults’ lives. Implications and considerations for future studies are discussed.

This article describes the results of a cross-sectional study of licensed clinical social workers’ (LCSWs’) views and behaviors related to integrating clients’ religion and spirituality in clinical practice.
