Biography

Paul Cowan is an assistant professor in the department of otolaryngology, or head and neck surgery, with subspecialty training and interest in rhinologic conditions which affect the nose and sinuses, the base of the skull and pituitary gland. He is actively involved in research projects including clinical trials comparing biologic drugs for sinonasal inflammatory disease and a grant exploring vitamin D as an inflammatory modulator in olfactory loss. He has completed prior research in the role of inflammatory cytokines in olfactory loss using a murine model and have examined the changes in the sinonasal mucosa over time of patients with chronic rhinosinusitis with nasal polyposis who have been on dupilumab therapy, using novel spatial transcriptomic technology.

Recurrent cerebrospinal fluid leaks require precise diagnosis, identification of risk factors, and tailored surgical and medical management that is patient-centered. The strongest evidence supports robust vascularized repair, intracranial pressure management in idiopathic intracranial hypertension, and specialized approaches in irradiated patients. Vasculopathy should not be overstated as an independent risk factor. Multidisciplinary care remains the cornerstone of durable outcomes and requires dialogue with appropriate colleagues.
