ChristianaCare one of only 44 nationwide to receive the distinction; one of only five to receive the highest level – Gold Recognition (WILMINGTON, Del. – Oct. 7, 2021) ChristianaCare has been named a recipient of the American Medical Association’s Joy in MedicineTM Health System Recognition Program. The distinction recognizes health systems that demonstrate a commitment to preserving the wellbeing of health care team members by engaging in proven efforts to combat work-related stress and burnout. The American Medical Association awarded ChristianaCare a “Gold Recognition” – the highest level within the program. Only 44 health systems nationwide received the Joy in Medicine distinction, with only four other health systems along with ChristianaCare achieving the Gold Recognition. “When caregivers feel supported and fulfilled in their work, the quality of both their experience and their patient’s experience improves,” said Heather Farley, M.D., MHCDS, chief wellness officer and head of ChristianaCare’s Center for WorkLife Wellbeing. “That’s why, when the COVID-19 pandemic struck, we deepened our investment of resources to support our caregivers. While there’s still a long way to go, we are grateful to receive this recognition as it affirms our commitment to helping our caregivers foster meaning, connection and joy in work and in life.” ChristianaCare received the “Gold Recognition” for achieving six criteria: An organization-wide commitment to improving physician wellbeing. An assessment of the costs of physician burnout. Evidence-based interventions to reduce and eliminate burnout. Leadership commitment to address areas for improvement. Interventions to improve teamwork. Structured programs that actively engage doctors to cultivate community at work. These criteria align with ChristianaCare’s commitment to support its caregivers in alignment with the organization’s core values of love and excellence. ChristianaCare has implemented structured ways of working that encourages leaders to create environments where their workers feel safe asking questions and feel empowered to identify and solve problems. ChristianaCare also uses numerous surveys and assessments to track progress in improving the wellbeing and experience of caregivers. ChristianaCare’s Unique Center for WorkLife Wellbeing ChristianaCare established the Center for WorkLife Wellbeing in 2016 to develop support systems to enable clinicians to reconnect with the joy and meaning in medicine. “We recognized the growing problem of stress and burnout in health care, and we made a decision to tackle it head-on,” said Janice E. Nevin, M.D., MPH, ChristianaCare president and CEO. “The work of our psychologists, researchers, clinicians and peer supporters in the Center for WorkLife Wellbeing is built on the understanding that when we care for ourselves, we are better able to care for others, resulting in decreased medical errors and a better experience of care for everyone involved.” The Center soon expanded its mission to support all of ChristianaCare’s caregivers, including those who don’t work in clinical roles. The Center also has received national recognition for its expertise and leadership in the field of workplace wellbeing. Throughout the pandemic, the Center for WorkLife Wellbeing has proactively shared a variety of resources to support caregivers as they work to provide safe, effective care and reduce the spread of COVID-19. This includes: The Care for the Caregiver program, which provides confidential individual peer support and group support to caregivers when they experience stress related to patient care or the workplace. Peer supporters and organizational leaders receive training so they can provide psychological first aid to caregivers whom they recognize are struggling as well as connect them to helpful resources if needed. Emotional support through mindfulness resources and regular rounds where the Center’s experts can check on staff wellbeing. OASIS Rooms, which are quiet sanctuaries inside the hospital where caregivers can go to take a break, de-stress, meditate or enjoy a chair massage. 24/7 mental health services and programs to help caregivers maintain healthy habits including proper exercise, sleep and nutrition. Fitness centers that are free for all caregivers to use and provide easy access to exercise and fitness programs. “I always use the analogy, you can’t take the canary out of the coal mine and teach it to be more resilient and then put it back in the same coal mine and expect it to survive,” Dr. Farley said. “You actually have to change the coal mine – the environment surrounding our caregivers and clinicians. That’s what we’re doing in the Center for WorkLife Wellbeing.” Solutions to a National Problem A national study examining the experiences of physicians and other health care workers who worked in health systems during the COVID-19 pandemic found that 38% self-reported experiencing anxiety or depression, while 43% suffered from work overload and 49% had burnout. “The COVID-19 pandemic has placed extraordinary stress on physicians and other health care professionals,” said American Medical Association President Gerald E. Harmon, M.D. “While it is always important for health systems to focus on the wellbeing of care teams, the imperative is greater than ever as acute stress from combatting the COVID-19 pandemic has contributed to higher rates of work overload, anxiety and depression. The health systems we recognize today are true leaders in promoting an organizational response that makes a difference in the lives of the health care workforce.” Launched in 2019, the Joy in Medicine Health System Recognition Program is a component of the American Medical Association’s practice transformation efforts, an ambitious initiative to advance evidence-based solutions to address the physician burnout crisis. Candidates for the program were evaluated according to their documented efforts to reduce work-related burnout through system-level drivers. Scoring criteria were based on demonstrated competencies in commitment, assessment, leadership, efficiency of practice environment, teamwork and support. It is in the news daily the awareness campaigns are everywhere – but for Spanish-speaking Americans, a lot of the information on COVID-19 and vaccination efforts against the disease weren’t reaching home. That was leaving a large swath of the population unprotected against the virus. Pastor Angel Maestre takes to the airwaves for his radio show, 97 Oasis, each week, speaking about faith to his Spanish-speaking audience. When COVID-19 vaccines rolled out, he began including teachings about health and science. “It was my duty as a leader,” Maestre says. Vaccination rates among the Hispanic population were extremely low at the time. In Richmond County, less than 10 percent of this community was vaccinated. Maestre partnered with Dr. Jose Vaszquez, an infectious diseases physician at Augusta University Health. Vazquez began appearing on Maestre’s radio show. He also spoke and answered questions at Maestre’s church, Centro Cristiano Oasis de Bendicion. “The Hispanic community has been one of the toughest communities to get vaccinated,” Vasquez says. “They’re not anti-vaxxers. But, most of the education put out there is in English. A lot of it is not in Spanish. So, they didn’t understand.” According to Vazquez, the top three concerns among the Hispanic community were that the COVID-19 vaccine contained fetal tissue, caused infertility and could result in undocumented immigrants encountering problems with immigration authorities. “They thought if we took their name and number, that ICE or immigration would come and take them, which is not true.” Vazquez eased concerns, and vaccination rates increased quickly. “I’m proud to say because of our outreach programs with some of our partners, specifically going through the churches, that it has actually increased to 30 to 35 percent.” Are you a journalist looking to know more about how COVID-19 is impacting Hispanic communities and what else can be done to ensure every person in America has access to vaccines? "We trust our doctors have been to medical school," Rettinger said. Type 2 diabetes: why treatment plans may need to be different for older adults

Around 6% of the world’s population suffer from type 2 diabetes. People of any age can develop the condition, but the number of older adults with type 2 diabetes is rapidly increasing worldwide. In fact, adults over the age of 65 now account for almost half of all adult cases. There are many ways type 2 diabetes can be managed – including controlling weight through diet and exercise, or taking a drug to manage blood sugar levels. But many people may not realise that type 2 diabetes in older adults can be more complicated to manage. This means people over 65 may need to be managed differently when it comes to type 2 diabetes. There are a number of reasons why type 2 diabetes may be more difficult to manage in older adults. First, ageing can affect blood sugar control, as the body’s organs (such as the pancreas, which controls insulin and blood sugar levels) lose their ability to work as well as they used to. On top of this, some research has shown that diabetes may cause people to age faster. It’s thought that this is due to high levels of sugar in the blood stream prematurely ageing the body’s cells. This premature ageing could lead to diseases associated with age-related decline (such as arthritis or dementia) happening sooner. Frailty – a state of health that is associated with reduced physical and mental resilience in older adults – also affects more people with type 2 diabetes than the rest of the population. In fact, an estimated 25% of older adults with type 2 diabetes are also frail. People who are frail and have type 2 diabetes have poorer health and increased risk of death from all causes compared to those who are not frail. Frailty is associated with reduced physical and cognitive functions and increased risk of low blood sugar. Both of these factors can make treating type 2 diabetes more complicated. Dementia, which is more common in older adults, may also make it more difficult to manage type 2 diabetes. This is because the memory problems this condition causes may make it harder for patients to remember to take their medication, or take the proper medication dosage. What’s more, type 2 diabetes in older adults is actually a risk factor for developing dementia – including Alzheimer’s disease. While the link between the two isn’t fully understood, elevated blood sugar levels and insulin not working properly have been suggested as causes. Having other health conditions can also make it more difficult to treat diabetes. Up to 40% of older adults with type 2 diabetes have four or more co-existing diseases – such as heart disease or dementia. These conditions can make it impossible to achieve normal treatment targets and the drugs used to treat them can interact with those used to treat diabetes – which could lead to harm if not managed carefully. Alongside this, poor access to proper medical care, and being more susceptible to low blood sugar in older age are also reasons why treating diabetes can be so difficult in this age group. Managing type 2 diabetes Most medical treatments for type 2 diabetes work to keep blood sugar levels low, and prevent them from spiking. But older adults with type 2 diabetes may actually have an increased risk of developing dangerously low blood sugar levels. This usually happens if the medication is not used at the correct dose, or in people who have had diabetes for a long time. Older adults can also be susceptible to low blood sugar levels. Syda Productions/ Shutterstock Having very low blood sugar levels is dangerous as it can increase the risk of falls – a serious and sometimes life-threatening problem in older adults. Very low blood sugar levels also increase the risk of heart problems. This means that healthcare professionals need to be careful they aren’t being too aggressive in treatment plans for older adults to avoid causing other health problems. Ageing may also alter the body’s response to low blood sugar. This is significant, as when blood sugar falls too low it is extremely dangerous and can even be fatal. Older adults may also be less able to recognise the symptoms of low blood sugar compared to young adults. This is because symptoms such as dizziness and confusion are often less specific in older adults, and can be confused with dementia. Older adults may also take longer to recover from low blood sugar. Given that repeated bouts of low blood sugar can mean that older people are less able to sense when it’s happened in the future, it’s important that drugs prescribed to older adults for type 2 diabetes are given at the correct doses. Care especially needs to be taken prescribing insulin, the body’s blood sugar control hormone, to very old adults as this significantly increases the risk of low blood sugar. Given our ageing population, it is projected that more older adults will have type 2 diabetes in the future. This makes it especially important to improve how we treat diabetes in this age group. Though specific treatment guidelines have been developed, some evidence suggests that care approaches need to be more cautious and personalised to each patient, taking into account their other health conditions, and that treatments consider quality of life for each patient. Dr Mark Prince Senior Lecturer in Mechanical Engineering Aston University New device created in Aston University lecturer's home during coronavirus lockdown Aston Business School expert explains how 3D printers are already used to make hearing aids, dental crowns and hips ‘Imagination is the only limit’ for additive manufacturing’s future, according to Dr Ahmad Beltagui A senior lecturer at Aston University is helping ear, nose and throat (ENT) clinics around the world by using the hi-tech ingenuity of 3D printing. Dr Mark Prince used 3D printing to prototype and produce a valve for a mask so that ENT surgeons could continue to examine and treat patients without fear of spreading the virus. Dr Prince, a senior lecturer in mechanical engineering at Aston University, was talking about his experiences in the latest episode of the 'Aston means business' podcast, presented by journalist Steve Dyson. The podcast also features Dr Ahmad Beltagui from Aston Business School, who talks about the wider benefits, some downsides and future potential of 3D printing in successfully disrupting traditional manufacturing. Dr Prince said his interest in additive manufacturing, or 3D printing, peaked after meeting two consultant ENT surgeons in the West Midlands. He said: "They were looking to solve a problem and I was able to use my additive manufacturing knowledge to help them out, and the result of that was that we developed a new technology and span a company out of it. "The company is Endoscope-i Ltd, founded in 2012 and now with an annual turnover of £200,000. The idea was simple: can we get medical endoscopic images of the ears and throat using a smartphone and we came up with a technique to do that." He said that, traditionally, he would have used large, expensive equipment to prototype the systems and new products. However, that wasn't possible with most of the university closed down at the height of the pandemic last year. He, therefore, had to look at other technologies that were safe to use in the home without breaking lockdown restrictions. Dr Prince explained: "When we are looking at diseases or concerns of the throat and nose we often stick an endoscope up the nose and down the throat, but of course you can't do that through a mask. "One of the founding members, Mr Ajith George, suggested that we could put a valve in the mask that allowed an endoscope to pass through it and allow the procedure to go ahead without breaking containment." "Work on the device started in mid-March 2020 and was ready for free supply to the NHS in November. Dr Prince added: "What that has allowed the NHS to do is any ENT clinic was able to fit it to a mask, put the mask on the patient, and carry on with an endoscopy procedure without all of the concerns of the pandemic." Meanwhile, Dr Ahmad Beltagui, a lecturer in operations and information management at Aston Business School, said you could get 3D printers from as little as £200 all the way up to expensive versions working with "exotic materials" like titanium. He said there were four main benefits to manufacturers: "Compared to traditional manufacturing methods you can produce shapes that are harder to produce; you can produce things in very small volumes; you can innovate faster and produce lots of prototypes, and you can produce things nearer to where they are required so you don't have to produce in a factory in one place and transport somewhere else." Dr Beltagui said: "The COVID-19 pandemic saw 3D printers used in homes to produce equipment such as cheap face visors for their local hospitals. The great thing about 3D printing is anyone can produce anything they want. "That's also the biggest risk – there is no control over what people produce or how safely they produce it. "At the beginning of the pandemic, someone in Italy found that they could produce some spare parts to keep the ventilators in their local hospital going. But then a few days later the manufacturer of those ventilators was considering whether or not it should take legal action." However, he said the printing machines had gradually become more reliable and there was a better level of quality, while the cost was coming down. Dr Beltagui added: "As to the future, the only limit is your imagination."

Orion Travel Insurance highlights how travelling around the world will be different With the recent announcement of relaxed quarantine restrictions for fully vaccinated Canadians, many eager travellers may be considering embarking on international travel as soon as possible. Orion Travel Insurance reminds travellers, that a Level 3 Global Affairs travel advisory remains in effect, and Canadians need to understand the ongoing uncertainty associated with international travel. “Entry requirements can vary by destination whether it be a different province, state or country. Travellers need to monitor those requirements closely and be prepared to carry additional documentation like proof of vaccination, and proof of valid COVID travel insurance,” says Kellee Irwin, Vice President, Orion Travel Insurance. “Many Canadians are excited to resume travelling, however a government advisory to avoid non-essential travel remains in place, and Canadians need to take the necessary precautions to ensure they protect themselves before embarking on their vacation.” With many Canadians now looking to book future vacations, Orion Travel Insurance wants to ensure travellers are protected. As of July 1st, Canadians are eligible for full emergency medical coverage with Orion Travel Insurance, even during a Level 3 COVID-19 related travel advisory, if they are fully vaccinated (having received all doses of a Health Canada-approved vaccine) at least 14 days prior to departure. Before planning any form of travel, Canadians should take steps to protect themselves and others. Canadians and permanent residents will be able to upload a copy of their vaccine certificates to the ArriveCAN app, allowing them to bypass mandatory hotel quarantine when they return to Canada. Travelling standards will vary by destination, and not all services or businesses may be open, or offer full services. Book your accommodations and activities before you go and check out their protocols in advance. Pack face masks, cleaning supplies, travel-sized snacks, water, and hand sanitizer – not only for the trip, but to have on hand at your destination. Monitor local media and government websites to stay informed of conditions and potential changes to entry requirements at your destination. Update your passport so it is ready when you are. And be prepared to pack additional documentation required for your destination, such as proof of vaccination, negative COVID test documentation, and proof of travel insurance. Insurance policies vary by provider. Review your policy and understand what you’re covered for before you travel. Don’t travel if anyone in your party or household is sick.

ADHD in adults: what it’s like living with the condition – and why many still struggle to get diagnosed

Many of us think of ADHD (attention deficit hyperactivity disorder) as a childhood condition – which is typically when it’s diagnosed. But a growing number of people are sharing their experiences of being diagnosed with ADHD in adulthood. Social media has even played a role in this, with reports of people going to see their doctor after first learning about symptoms on TikTok. In fact, around 2.5% of adults are thought to live with ADHD – including us. Yet despite this growing awareness, many adults continue to struggle to get a diagnosis. ADHD is a genetic neurodevelopmental disorder, in which the brain grows differently, lacking action from specific chemicals involved in pleasure and reward. This means ADHD brains often search for ways to stimulate these chemicals, which is why people can experience inattentiveness, hyperactivity and impulsivity. Common traits of ADHD include: Not following through on longer tasks (or not starting them) Getting distracted by other tasks or thoughts Seeking out risk or activities that provide immediate reward Restlessness (either outwardly or internally) Interrupting other people (without wanting to) Symptoms are similar for both adults and children, although elements of them differ or change as we age. For example, inattention is the most persistent symptom in adults. ADHD can be debilitating and is associated with higher likelihood of lower quality of life, substance use issues, unemployment, accidental injuries, suicide and premature death. In addition, ADHD can cost adults around £18,000 per year because of things like medical care or paying for social support. It’s also commonly associated with a wide range of co-existing conditions in adults. For example, depression is almost three times more prevalent in adults with ADHD. And nearly half of all adults with ADHD also have bipolar spectrum disorder. Around 70% of adults with ADHD also experience emotional dysregulation, which can make it more difficult to control emotional responses. It’s also thought that almost all adults with ADHD have rejection sensitive dysphoria, a condition where perceived rejection or criticism can cause extreme emotional sensitivity or pain. On top of this, adults with ADHD may have poor working memory – such as being unable to remember a simple shopping list – and “time blindness” (the inability to perceive time). Some may also have oppositional defiant disorder, which means they often react poorly to perceived orders or rules. While none of these co-existing conditions are used to diagnose ADHD, they can make ADHD feel all the more difficult to live with. Being diagnosed Getting an ADHD diagnosis as an adult in the UK is notoriously difficult – with reports of some people waiting up to five years. Waiting for a diagnosis is common for adults with ADHD. Roman Kosolapov/ Shutterstock This is because you can only be diagnosed by a specialist psychiatrist. But even with a referral to a specialist, a person has to show clear evidence of almost all ADHD traits, having had these traits since childhood, and that they’re having a serious affect on their life – such as causing issues with work, education, or maintaining relationships. For us, our experiences of being diagnosed with ADHD aren’t all that different from what other adults have gone through. Like many people I (Alex) was only diagnosed with ADHD “by accident” after being referred to an NHS psychiatrist to get help with (what I now know to be) alcohol self-medication. Because of my ADHD, my brain demands quite extreme inputs most of the time. Ironically, I’ve published scientific papers on ADHD and – probably due to a classic ADHD lack of self-awareness – it didn’t cross my mind that I could have it. The “label” has since helped me move away from feeling broken toward an understanding of my behaviour. My main challenges remain prioritising tasks based on importance (instead of excitement) and quite extreme anti-authority behaviour (sometimes called oppositional defiance). I am also a terrible spectator, struggling to attend conference talks or sit still at the theatre – it can feel like physical pain. On the other hand, I (James) was diagnosed pretty quickly because I used a private clinic – though there was still a long wait for medication. Yet I’d known for five years before this that I probably had ADHD, but coped with it well until the pandemic. The added pressure of isolation and increased workload impacted my mental health, so I sought a diagnosis. Now diagnosed and medicated, life is getting easier to cope with – although there are still many challenges every day. I frequently get anxiety about the silliest things, like talking to a friend, but appearing on television is fine. On a daily basis I forget many simple things, such as where I left my keys, or that I am running a bath. I struggle immensely with controlling my emotions and with rejection especially. For example, when no one responded to a joke I made about my ADHD on a senior management messaging group I was tempted to quit my job. I am utterly unable to pay full attention in meetings or seminars and cannot control my impulse purchasing. While there’s a growing recognition of ADHD in adults, many people still live with it undiagnosed for any number of reasons – sometimes even because they’re unaware that what they experience is actually different from other people. Understanding the condition in adults, taking it more seriously as a disorder, raising awareness of it, and investing in services to improve diagnosis times are key. Diagnosis opens the door to treatment, which can have a marked impact on living with the disorder – such as improving self-esteem, productivity and quality of life. Cyber attacks and ransomware are incidents that are happening more and more often. They are threatening America’s energy supply, food chain and critical infrastructure. Recently, two Georgia hospitals were victims of a recent attack. Media from across the country asked experts like Frank Katz, director of Georgia Southern University's Center for Applied Cyber Education, who is vulnerable and why. Katz, who directs the Center for Applied Cyber Education at Georgia Southern University’s Armstrong Campus in Savannah, said hospitals can be targets for extortion as well as sources of personal identifying information that can aid in further fraud. “They are more likely to pay than another type of business because it’s a life and death situation,” Katz told WSAV-TV. “It really has become a situation of money, pure and simple.
