The forgotten crisis – are opioids and overdoses being overlooked during the COVID-19 outbreak?
As one crisis overcomes another in the spectrum of news coverage and public health messaging, there’s a serious concern that drug users are particularly vulnerable and potentially being forgotten in the wake of COVID-19. With millions of Americans forced into weeks of extended isolation, several communities have reported a spike in drug overdose deaths, prompting health officials to raise concerns about the safety of those suffering from substance use disorders amid the COVID-19 pandemic. In Jacksonville, Florida, the fire and rescue department reported a 20% increase in overdose emergency calls in March. In Columbus, Ohio, the county coroner’s office saw a surge in overdose deaths, including 12 in a 24-hour period the first week of April. And in New York State, at least four counties have acknowledged an increase in reported overdoses, including Erie County, where officials saw at least 110 drug overdoses, including 36 deaths, reported since the beginning of March. “The opposite of addiction is not sobriety but connectedness,” said Dr. Joseph Hernandez, an associate professor in the Department of Psychiatry and Health Behavior at the Medical College of Georgia at Augusta University. “As we practice social distancing to control the spread of COVID-19, many addicts have lost their traditional in-person meetups, which may explain the jump in overdose deaths.” Despite these challenges, Hernandez says the addiction recovery community is working to maintain connections by switching to online or virtual formats. Additionally, most rehabilitation centers for substance abuse have remained open and are screening their residents to prevent the spread of COVID-19. It’s unclear whether the reports from local officials reflects a broader trend nationwide. Drinking alcoholic beverages may be more appealing amid unease about the coronavirus, as people deal with shelter-at-home orders, fears about the economy and boredom, says a Baylor University researcher who studies alcohol use and misuse. But with regulations providing less access to alcohol, this may be a good time for individuals struggling with alcohol use to begin recovery and for others to guard against over-relying on alcohol or other substances. When bars and restaurants began closing — other than for such options as pickup, delivery or drive-through — liquor stores saw a surge in business, according to news reports. But Pennsylvania closed its liquor stores — some people defied stay-at-home orders and drove to liquor stores in neighboring states — and New Hampshire recently closed some of its liquor stores, according to reports. Other states who deemed the businesses “essential” also may take another look at the issue. How to grapple with the risks of substance use and misuse during this stressful time is the subject of this Q&A with Sara Dolan, Ph.D., associate professor of psychology and neuroscience at Baylor University, who has done extensive research on substance use and misuse. Q: There are memes — some of them humorous — going around about heavy alcohol use during quarantine. Why might people be drinking more than usual? DOLAN: People have many different motivations for drinking, and I think self-isolation amplifies some of those. First, people drink to feel good. For some, being out of the normal work routine may feel like a time to let loose. People also drink to feel less bad. It would be normal to feel out of sorts now that we are social distancing. It also would be normal to feel some boredom, and certainly we feel anxiety and uneasiness about our current circumstances. Alcohol may be seen by some as a way to cope with those negative feelings. Q: How might the “new normal” be especially hard for alcoholics? Could this be a time to begin recovery? Some may be social distancing from drinking buddies, although that wouldn’t stop drinking alone. DOLAN: A forced lack of access to alcohol through social distancing and bars being closed can be a great jumping-off point for someone to begin recovery, especially when people are physically distancing from the people they drink or use with. But this can be an especially tough time for people because they may not be able to cope with all the new stressors, especially if they don’t have access to their typical means of coping. For example, for someone who usually relies on friends and family for support, social distancing can cause more stress. And loneliness is especially difficult when it is stacked on top of economic, illness and other anxieties we are experiencing. It is important for us to reach out, from a distance, to family and friends and other resources to help us cope positively so we don’t turn to drinking or other drug use to help us cope. I worry about people who are very heavy drinkers who suddenly stop drinking. Alcohol withdrawal, which can happen when a very heavy drinker stops drinking suddenly, can be very dangerous. Symptoms include anxiety, shakiness, sweatiness, headaches, nausea and even hallucinations – seeing and hearing things others don’t see or hear — and seizures. If someone who usually drinks very heavily and suddenly stops drinking experiences these symptoms, immediate medical attention is necessary. Q: What strategies would you suggest as far as dealing with heavy drinking during this time – both for drinkers and for those who love them? DOLAN: We really all need to be compassionate toward one another, regardless of our individual struggles. This is a difficult time for everyone – it is normal during a crisis like this to feel anxious and even depressed. Support is very important, both for those who are struggling and for those who seem like they are doing fine. This support can take a lot of different forms, from offering an ear to listen to offering specific strategies, such as mutual recovery groups (such as Alcoholics Anonymous, offered online) and other coping resources, like apps. Artificial Intelligence Playing a Powerful Role in Understanding and Fighting COVID-19
Artificial intelligence is emerging as a powerful new ally in tracking COVID-19, modeling the virus at the molecular level, and analyzing the myriad research results being published daily. James Hendler, the Tetherless World Professor of Computer, Web, and Cognitive Sciences at Rensselaer, and director of the Rensselaer Institute of Data Exploration and Applications, is leading campus efforts to marshal AI resources for the purpose of battling the virus. “The bottom line is that, at heart, dealing with COVID-19 is a ‘big data’ problem, and AI is a crucial tool in the big data toolkit,” Hendler said. For example, IDEA and the Rensselaer Libraries have collaborated to maintain lists of COVID-19-related data sources and scholarly research publications. AI is being used to translate literally thousands of scientific insights from text-based research products into forms that can more easily be analyzed. Hendler and other Rensselaer AI experts are also involved in studying the spread of the virus under different policy measures at the local, then state and national, and ultimately global scale. Additionally, Rensselaer is part of the national COVID-19 High Performance Computing Consortium, which is offering researchers access to supercomputers for COVID-19 research. In addition to AiMOS, the most powerful supercomputer housed at a private university, Rensselaer is offering access to the expertise of world-class faculty, including in artificial intelligence. Hendler said AiMOS is one of the few facilities that can truly offer a platform optimized for artificial intelligence computing. “AI has helped us achieve an excellent understanding of the coronavirus and its interactions at the molecular level. That’s going to make it possible not only to model the virus, but also how it will interact with potential drug targets and vaccines,” said Hendler. “A platform like AiMOS is invaluable for molecular modeling in drug discovery, helping scientists cope with a huge and rapidly changing literature, and exploring means to model, and then mitigate, the spread of the disease. These are the kinds of things that modern AI can do.” Hendler has authored over 400 books, technical papers and articles in the areas of Semantic Web, artificial intelligence, agent-based computing and high performance processing. One of the originators of the “Semantic Web,” Hendler is the former Chief Scientist of the Information Systems Office at the U.S. Defense Advanced Research Projects Agency (DARPA) and was awarded a US Air Force Exceptional Civilian Service Medal. Among other organizations, he is a member of the National Academies Board on Research Data and Information, a fellow of the National Academy of Public Administration, and he serves as chair of the Association for Computing Machinery (ACM) U.S. technology policy committee.

This Video Game Can Teach You How to Bring a Vaccine to Market
Getting a drug or vaccine from the research bench to the bedside of a patient in need is a complex process, and one that researchers around the globe are currently trying to navigate as quickly as possible to address the spread of COVID-19. To improve understanding of this intricate, interdisciplinary undertaking, faculty and students in the Games and Simulation Arts and Sciences (GSAS) program at Rensselaer Polytechnic Institute, in collaboration with the Icahn School of Medicine at Mt. Sinai Hospital, are developing a video game called "Cure Quest." This game will help bridge the gap in med school education between the clinical side of medicine and basic research. “Cure Quest will allow medical students to tie together the necessary steps for vaccine development in dramatic cases like we’re seeing right now with COVID-19,” said Ben Chang, a professor of arts and Director of GSAS at Rensselaer, “or in drugs we would use day-to-day.” The main character in the game is sent to an island where there is a new disease and must overcome obstacles in their pursuit of creating a new drug to cure the disease. When its development is complete, Chang envisions Cure Quest being used by the general public, as well by students in scientific and medical fields, to better understand the drug development pipeline. It’s lawsuit after lawsuit after lawsuit as blame, accountability and, eventually, restitution is sought as the opioid epidemic's damage is sorted out across courtrooms in America. The latest turn in the road is that major drug store chains are now suing doctors in the bell-weather area of northeast Ohio with a claim that it is those with the pen and prescription pad that need to own the responsibility for the rampant opioid abuse across the country. “CVS, Walgreen Co., Walmart, Rite Aid and other major pharmacy chains said opioid prescribers bear responsibility for the prescription narcotic crisis, but unlike the drugstores, have not been sued by Cuyahoga and Summit counties. Recent media coverage is pulling back the curtain to reveal another angle on opioid abuse. The facts are startling, and it seems there is a direct relation between those facing economic stress and hardship and abusing opioids. “In 2015, Jennifer Silva, a professor of sociology and anthropology at Bucknell University, began interviewing people in the coal region of northeastern Pennsylvania. She was working on a project, which would become the book We’re Still Here, about how poor and working-class Americans were affected by the collapse of the coal industry—the major job provider in the region. She was curious how the regional decline might have shaped her subjects’ politics. But she quickly noticed a startling trend alongside the growing unemployment: Her subjects and their families were struggling with opioid abuse. At community meetings, doctors and coroners would debate solutions to the problem. Should they be arresting people? Should they be creating support groups? She describes one desperate parent who asked whether Donald Trump’s proposed border wall would keep black tar heroin from getting to Pennsylvania. Silva’s interviewees might have been representative of an awful connection between job loss and opioid abuse, a connection that continues to be bolstered by research. Media Training Can You Afford NOT to Be Prepared When Reporters Come Calling?
Want to see someone in a respected position of power throw away a career in all of about two minutes? Stephen Duckett was an expert in his field. Respected globally. Until this… Watch this exchange Duckett wasn’t prepared to deal with media. He paid a serious professional price and left a lesson for us all to learn from. In fact, his exchange is used by a lot of media trainers on exactly what not to do in a crisis situation. As a former news producer, I remember scrambling to find an expert opinion or perspective when news was breaking. If a story required explanation and when it was beyond the grasp of even the brightest of reporters we needed the help of supporting sources. They were critical in helping explain, validate, and ideally, break down just what was going on to our audiences at home. When an outbreak struck, we needed an epidemiologist. When a rare earthquake shook the region, the search for a seismologist was on. Finding the expert wasn’t usually the problem. We had local universities, colleges, and health care facilities within arm’s reach. However, finding an expert who could effectively speak on camera often was. Our goal was to keep people from turning the channel – so boring, highly technical speaking experts simply wouldn’t do it for us. When I worked in TV, we needed someone who could explain a complex subject, in layman’s terms, and in sound bites of about 15 seconds. It may not seem like much, but it is a lot to ask. Explaining high level content is not easy, but the reality is that your audience needs complex subjects explained in the simplest of ways. To do this right, it requires understanding the roles and goals of media and media training. Anyone who will be representing an organization, institution, or corporation needs some form of coaching. It’s a must-have requirement for most CEOs and politicians. Turn on your news at just about any hour of the day, and you can easily tell who has been properly prepared and trained and who hasn’t. It doesn’t happen organically. It takes work. It takes expert coaching. Media-friendly speakers bolster credibility and get noticed. In times of a crisis, a trained speaker may be the missing piece that can make or break a situation a reputation. Those who are not trained stick out—in a very bad way. They stammer, evade questions, ramble and are usually incoherent. If the task is mainly to describe a current situation, an untrained expert may ramble or speak hundreds of feet over the head of the average person. If there’s a crisis at hand—a tragedy, a scandal, a lawsuit or worse—an untrained speaker risks making a bad situation worse. Take a look back to July of 2013—an out-of-control train carrying crude oil exploded, destroying the downtown section of Lac Megantic, in Quebec, Canada. Thirty buildings were leveled, killing 47 people. In this small town, everybody knew somebody who was killed. Edward Burkhardt, president of Montreal, Maine & Atlantic Railway Inc. arrived shortly after the explosion. He was the near-perfect storm—an untrained, and likely uncoachable leader. He was clearly in distress, rattled, arrogant, and unprepared. He fixed blame at the worst possible time, blaming the train’s engineer and the local fire department. A journalist asked how much he was worth. A town is destroyed, the world is watching, close to four dozen people are dead, and his reply was, “I’m worth a lot less now than I was last week.” Proper media training and coaching would not have prevented the horrible tragedy, but knowing how to speak during such a high-stress situation and knowing what questions to expect might have mitigated the visceral reaction of residents, industry and government officials. As it stands, Edward Burkhardt and the company, no matter how successful either had been in the past, are now forever associated with that interview. Conway Fraser worked for the Canadian Broadcasting Corporation for almost 20 years. He’s a Gemini award-winning journalist who worked as a national reporter and investigative journalist. These days, he specializes in strategic and crisis communications as well as media coaching. Conway’s worked with some of Canada’s top corporate executives, academic leaders, and politicians. In both journalism and in PR, he has seen executives who have spent decades building a reputation only to have it destroyed in moments because they weren’t prepared to deal with the media. They’ve either never received media coaching or, Conway says, have taken mandatory training but were never further invested in. In other words, he says, they thought they didn’t need media training and were only taking it to appease the Board or ownership. “Proper media coaching isn’t just about knowing how to spew a main message, use effective body language or bridge away from challenging questions,” Conway says, “In my sessions I also teach my clients about the media, what their job is like, what a day is like for a reporter, what they need from you and how to respect their role. If anyone doubts the value of that, they’re playing with fire. Ask Stephen Duckett.” Natalie Duddridge is a reporter for NY1. I worked with Natalie years ago in Canada at the start of her career. Natalie’s talent as a journalist has taken her to Ottawa (Canada’s capital), Toronto, and most recently, New York. New York sits at the top (sorry L.A.) of media markets in the United States. It has a huge audience of about 10 million people and it is hands down the most competitive media game on the continent. Reporters in these markets never get to rest. They’re either chasing stories, or when they are covering a story they had better have an angle, source, or expert that the others do not. And when you are in a race with at least six other news outlets, standing out isn’t easy. Finding experts and having them ‘camera’ or ‘microphone’ ready is a must. As Natalie explained to me, getting an expert for a story is absolutely crucial. In a market as diverse as New York City, those experts can range from police to politicians, to health and education officials, to scientists and artists. Natalie also lent some great perspective on just how a reporter puts together a story and how your expert needs to know that news is also about storytelling and not just details. “Last week I did a story about the heroin epidemic plaguing the Borough of Staten Island,” Natalie explained. “We like to begin our stories with a human element, in this case a recovering drug addict. In addition to his personal opinions and insight about how to prevent and slow the opioid crisis in the region, we also reached out to the local Drug Rehabilitation Center and interviewed a doctor as well as a therapist. We also interviewed the Staten Island District Attorney about the work he’s doing with New York State Governor to get more dollars for a drug task force to do everything from put more money into preventative education, build rehab centers, add police, fund additional assistant district attorneys to process criminal cases.” If you were counting, you can see she spoke with three key experts on the subject of the story. Each was essential in the reporting process. “For this story we featured several different experts on varying opinions on how this current drug crisis should be dealt with. All of the facts, stats and data for this story were based on the officials and experts we contacted.” Being prepared and knowing what the reporter wants are also ideal elements in conveying the right message. For TV, short, smart, and to-the-point responses work best. “We need a 10 to 15 second sound bite that is ideally informative and clear, and in some cases entertaining,” Duddridge told me. “When I am making calls to experts, I essentially do a pre-interview over the phone to hear how clearly they can explain a topic. If they are great at breaking down studies and terms into focused ideas that are concise and fit into a two-minute story, our job as reporters is so much easier.” A win/win scenario. Your message is delivered, your institution and experts are promoted and the reporter files a great story. It all seems simple. But it’s not. It takes training and preparation. If you are going to offer up your experts for an interview, they need to be ready. Media training takes time, it costs money – but it’s an investment in your staff and your institution. As we learned from the example above, not knowing how to answer, interact, and respond to media can be devastating. Performing under pressure only succeeds with practice and training. If your experts are media trained the reward will always outweigh the risk. Here are a few tips: Get media trained – Use a professional media coach. It costs money, but the professional development, readiness, comfort, and ability to deliver will pay off ten-fold. Media-friendly experts get noticed – When your expert is on the news, people see them. They are representing your institution, so think about what this means for your credibility and recruiting potential. You never get a second chance at a first impression – An old, clichéd saying, but it’s true. A weak speaker who comes across poorly imprints a negative impression on viewers, peers and your institution as a whole. It’s amateur hour, and it doesn’t need to be. Friends for life – Once you prove yourself as a worthy and media-friendly source, the media will keep coming back. Experts who can provide journalists with the information, perspective, and sound bites they need are not only appreciated but noticed and remembered by all media. Once you have established yourself as a reliable source, expect the reporters to come calling time and time again. Dividends – Every story where your expert looks good is positive earned media. A Lego-Like Approach to Improve Nature’s Own Ability to Kill Dangerous Bacteria
The Centers for Disease Control and Prevention considers antibiotic resistance one of the most urgent public health threats, one that affects communities worldwide. The ramifications of bacteria’s ability to become resistant to antibiotics can be seen in hospitals, public places, our food supply, and our water. In their search for solutions, researchers at Rensselaer Polytechnic Institute have been looking to nature. In a paper recently published in Biomacromolecules, the team demonstrated how it could improve upon the ability of nature’s exquisitely selective collection of antimicrobial enzymes to attack bacteria in a way that’s much less likely to cause bacterial resistance. “The idea is that we could take nature’s approach and just make it better,” said Jonathan Dordick, a chaired professor of chemical and biological engineering and a member of the Center for Biotechnology and Interdisciplinary Studies (CBIS). In order for bacteria to grow and live, they naturally produce autolysin enzymes that can break down their own cell walls, allowing those cells to divide and multiply. In attacking one another, bacteria take advantage of a similar process, using an antibacterial protein known as a bacteriocin to kill a bacterium. Bacteria can also be attacked by bacteriophages, which are viruses that infect bacteria. They produce phage endolysin enzymes, which attack the bacterial cell from the inside. All three types of enzymes are broadly known as cell lytic enzymes, as they catalyze the breakdown of the bacterial cell wall. “It’s very difficult for bacteria to become resistant to the action of these enzymes,” Dordick said. “For example, if they became resistant to an autolysin, they wouldn’t divide.” Like building blocks, most cell lytic enzymes are modular. They’re made up of one binding domain which attaches to the cell wall, and a catalytic domain that breaks holes in the cell wall — effectively destroying the targeted bacteria. “The idea was: Could we use a Lego-like approach here? Could we take a binding domain from one enzyme and can we mix it with a binding domain or catalytic domain of another one?” Dordick said. The issue of antibiotic resistant bacteria and disease is a serious one and of great concern to the medical community. Will e-cigarettes and vaping be the next addictions epidemic to sweep across America?
In a recent op-ed, David T. Courtwright, Ph.D., an author and addiction specialist opined that there’s a market to getting people hooked on substances and that e-cigarettes are the next big problem facing America. "I had just finished a new book on addiction when the vaping crisis erupted. The gist of the book is that that globalization, industrialization, mass marketing, digitization, and social media have turned the ancient human preoccupation with disreputable, potentially addictive pleasures into lucrative, commercially normal enterprises. Bad habits have been McDonaldized. Vaping couldn’t have been a more perfect example of this. I call those who help make bad habits routine “limbic capitalists,” a reference to their products’ neural common denominator. Whether they sell junk food, porn, slots, computer games, alcohol, or drugs, they target the limbic system, the brain networks responsible for pleasure, motivation, long-term memory, and other survival functions linked to emotions. Biological evolution shaped the limbic system, which is indispensable for life and reproduction. But cultural evolution and technological change created a trapdoor. The same neural pathways can be exploited — lethally — by entrepreneurs of brain-rewarding products that foster excessive consumption and addictive behavior." Gene therapy and the next frontier of medicine
Genetic testing today is mainstream, marketing to consumers who want to know where in Europe they came from or what types of hereditary diseases they could develop. For around $200 you can trace your family tree to learn your origins or identify genetic abnormalities that could signal disease. James Dahlman, assistant professor in the College of Engineering’s biomedical engineering department, specializes in genetics and believes these genotyping services can be helpful, as long as they are used responsibly. “If you’re going to start making medical predictions, you have to be careful,” said Dahlman. “Most people are not equipped to interpret statistics correctly, which can lead to negative predicting and ethical dilemmas. In a few years, genetic counselors will be in high demand so folks can make better decisions about their health.” Dahlman is fascinated by genetics, citing gene therapy as the most interesting field in the world. And it’s a field that he is revolutionizing through his research. Gene therapy is an experimental technique that uses genes to treat or prevent diseases, including hemophilia, Parkinson’s, cancer and HIV. It can help manage a number of diseases by leveraging genes instead of drugs or surgery. Although gene therapy shows promise, there are still risks involved, including unwanted immune system reactions or the risk of the wrong cells being targeted. That’s where Dahlman’s research comes in. Dahlman’s lab focuses on drug delivery vehicles, which are nanoparticles. The nanoparticle delivers gene therapies to the right place in the body to fight disease. It’s critical that the gene therapies only target the unhealthy cells to avoid damaging healthy ones. Dahlman is laser focused on ensuring the nanoparticles know what paths to take to reach the correct organ to start the healing process. “The issue with genetically-engineered drugs is that they don’t work unless they get to the right cell in the body,” said Dahlman. “You can have the world’s best genetic drug that's going to fix a tumor or eradicate plaque, but it’s not going to be effective unless it travels to the right organ.
