Community pharmacy can play a ‘key clinical role’ in delivery of COVID-19 vaccinations – new research

New research published in BMJ Open shows that community pharmacy could play a ‘key clinical role’ in the future role of COVID-19 vaccination programmes, according to a study led by Aston University in Birmingham, UK, in collaboration with UK and international researchers. The team found that community pharmacists, as a ‘skilled clinical workforce’, could positively contribute, supporting the community in which they serve - by playing a critical role in ongoing COVID-19 vaccination campaigns. The researchers working on the PERISCOPE study found that community pharmacy is uniquely placed to support individuals, because it is seen by the public as a credible, trustworthy service, which could be key to any future clinical role it might play, especially where addressing vaccine hesitancy in ‘seldom heard’ communities. They are therefore calling on decision-makers to endorse and provide their support for a public health role for community pharmacy. Across the UK, community pharmacy is a critical part of primary care. According to the Kings Fund, as of the end of March 2019, there were more than 11,500 community pharmacies in England alone. It is viewed as one of the four pillars of the primary care system, along with general practice, optical services and dentistry. It has also, in areas of the UK, helped to deliver COVID-19 vaccinations. The study included partners from the Universities of Sheffield, Oxford, Hull and Bradford in the UK, as well as internationally, the University of British Columbia and University of Tasmania. The group reviewed more than a hundred documents including peer reviewed articles, blogs and websites on the role of community pharmacy during COVID-19 and other previous pandemics. Their findings were discussed with more than 30 health professionals and members of the public, to ensure that the findings made sense in the real world. Health professionals included pharmacists, pharmacy technicians, dispensers, counter assistants, and GPs, together with members of the public from a range of diverse ethnic backgrounds. Several recommendations were made by the researchers from the findings of the study. Most significantly the group found it was imperative that policy and practice should focus on the clinical role of community pharmacy. Dr Ian Maidment, reader in clinical pharmacy at Aston University and former community pharmacist leading PERSICOPE, said: “We need to use community pharmacy to a much greater extent for COVID-19 vaccination, particularly for boosters against new variants such as the Delta (Indian) variant. The current model (for example, the large hubs) may not be sustainable in the longer term, particularly if annual COVID-19 vaccination is required. “Our work found some key ways to make this happen. The easy access and local convenience of high street pharmacies makes them an ideal location for vaccinating at-risk populations.” The study includes guidance for policy makers: • Have a clear role for community pharmacy in response to the public health agenda, with that role championed by decision-makers • Involve frontline community pharmacists in the development of policy and service specification in relation to vaccination • Provide prompt, clear, consistent guidelines with adequate detail and enough flexibility to allow community pharmacies to adapt the guidelines to meet the needs of their local population • Provide adequate funding and reimbursement for the delivery and necessary adaptations of any new services community pharmacies are asked to deliver • Provide pharmacy teams with adequate systems to deliver this new role and then trust them to deliver. Hadar Zaman, head of pharmacy and medical sciences at University of Bradford and a community pharmacist, said: “Our research has highlighted the important role community pharmacy has played in overcoming vaccine hesitancy, particularly in ethnic minority communities who have been disproportionately affected by COVID and subsequent mortality. “What comes out very strongly, especially in areas of high social deprivation, is that community pharmacists have worked very closely with their local communities addressing concerns around vaccine safety. “It is through these strongly rooted relationships in local communities that we will ensure vaccine uptake rates in ethnic minority and the wider population can be further improved. Therefore, community pharmacy needs to be seen as an essential delivery partner if the Government is to achieve its national vaccination coverage in the short and long term”. PERISCOPE searched for the best evidence across the world and the team included international collaborators. The findings therefore have international relevance. Maura MacPhee, professor of nursing, University of British Columbia and member of the research team, said: “Our review findings and recommendations for decision-makers, community pharmacists and pharmacy users are adaptable and relevant internationally, including my country, Canada, where community pharmacy has a major role to play in COVID-19 vaccination programmes.” Juanita Breen, also a member of the PERISCOPE team and associate professor of dementia studies at Wicking Dementia Centre, School of Health and Medicine, University of Tasmania, added: "This study demonstrates how pharmacists can contribute towards this important public health initiative and enhance the uptake of the vaccine. “It provides important learnings for other countries on how best to utilise the skills of our most accessible health professional - the community pharmacist." Professor Claire Anderson, chair of the Royal Pharmaceutical Society’s English Board said: “This research clearly demonstrates the vital role community pharmacy has played during the pandemic, providing essential advice to communities and tackling health inequalities in areas of high social deprivation. “Policy makers and commissioners need to take forward the recommendations of this research and ensure the strengths of the community pharmacy network are maximised for the benefits of patients.” Alastair Buxton, director of NHS Services at the Pharmaceutical Services Negotiating Committee, said: “This research provides a timely examination of the role community pharmacy teams have played in supporting their communities to fight back against COVID-19. “By keeping their doors open throughout, pharmacies have maintained day-to-day activities, and managed increased demand for many services - including advice on the management of minor illness. They have also substantially increased the number of flu vaccinations administered and played a key part in the COVID-19 vaccination programme. “These findings will help guide policy in the later stages of the pandemic and guide practice in any future pandemics.” Tony Kelly, a diabetes ambassador, Diabetes Strategic Patient Partner - NHS Birmingham and Solihull Clinical Commissioning Group and member of PERISCOPE, said: "Community pharmacists are ideally placed at the forefront of the vaccination agenda as they are the nucleus of ethnically diverse communities and are often the first point of contact for most people." PERISCOPE was jointly funded by UK Research and Innovation (UKRI) and the National Institute for Health Research (NIHR).

Georgia Southern grad student researches different planting treatment effects on Tybee Island sand dunes

The sand dunes of Tybee Island have taken a beating in recent hurricane seasons. Georgia Southern University biology graduate student Shannon Matzke is researching ways to combat the destruction of the dunes for her restoration project by collecting data on the effects of various planting techniques to see which technique best increases the longevity of the dunes. As a part of the project, Matzke collects monthly data on the growth and survival of plants, as well as on the sand accumulation associated with different planting densities and groups of species. She also spends time analyzing data, exploring new techniques and tools to use in the field, researching dunes and vegetation, and conducting outreach for the project. “Working on the beach has been such a fun experience,” Matzke said. “It has also been rewarding to learn new techniques and how to use new equipment to answer more questions about the success of the restoration project. Perhaps my favorite part though has been the outreach associated with my project. The overall dune construction and beach nourishment is so important to island locals, and I’ve enjoyed being able to explain how my work fits in with the larger project.” Of the plants surveyed, 95% survived the first growing season. On average, plants doubled in height and canopy cover when compared to their size when initially planted. Matzke is also collecting data in areas of the constructed dune that were left bare to compare against the vegetated sites. Unlike the vegetated dunes, bare sites are experiencing erosion, which further relates the importance of adding native vegetation to any new dune construction. Matzke said she was drawn to Georgia Southern and the biology department because of proximity to the coast. She also appreciated her advisor Lissa Leege, Ph.D., who has worked with native and endangered plant species. “I am so glad that I chose to earn my master’s at Georgia Southern because I have been able to make connections with researchers inside and outside of my department, as well as with industry and government professionals,” she said. “I have also enjoyed being able to work on Tybee and the convenience of having the Armstrong campus close to where I live in Savannah.” Matze, who will graduate in fall 2021, hopes to continue her work with native plant ecology. “After I graduate from Georgia Southern, I’d like to work for a state or local agency continuing in the natural resources management field,” Matzke said. “I am interested in continuing working on invasive species research, endangered plant conservation and habitat restoration using native species. Pothole project to pave way to smoother roads

• Joint research project will combine effort and expertise of pavement engineers, material scientists and computational fluid dynamics experts • Project to look at improving quality, longevity and accessibility of the highway network • Aston University will be working with University of Nottingham and seven other industrial partners including Highways England and the Road Surface Treatment Association A project which will be the driving force behind the redesigning of roads and changes in road repairs could revolutionise the way potholes are repaired, and road surfaces are designed. The three-year collaboration, ‘Prevention and Management of Road Surface Damage’, is being led by Mujib Rahman, professor in civil engineering, Department of Civil Engineering at Aston University and Dr Nick Thom of the University of Nottingham. The project will combine the efforts and expertise of pavement engineers, material scientists and computational fluid dynamic experts from both universities. The research aims to improve the design and construction of roads to minimise surface damage caused by water freezing and thawing, and general wear and tear from traffic. It also will be looking at how to change the science behind road repairs and maintenance. Research will be looking at two main elements: - Enable the design of roads to prevent surface damage from water and environmental factors - Introduce a change in the management of road repair and create a more durable repair of the road surface. . Professor Rahman said that smoother roads were critical to the nation as the 250,000 miles of paved road, valued at £750 billion, which is the network for deliveries of goods and services across the UK. But he added potholes and damage to roads had become ‘increasingly problematic’ for all users. “We want to drive out the pothole epidemic that has overcome the UK in recent years. Due to record breaking rainfall, extreme cold weather and tight financial constraints on highway authorities, this situation has become much worse. This has been combined with the lack of longevity in some repairs” he said. Dr Nick Thom, from the Nottingham Transportation Engineering Centre Research Group, said: “The expected deliverables are material specifications and maintenance guidelines designed – like a Covid vaccine – to stop this disease of the road in its tracks. In the main this will be about doing better with currently-used resources, but, given the expected slump in future oil production with consequent loss of bitumen supplies, the search will also be on for alternative viable high-performance products.” The project has been funded by Engineering and Physical Science Research Council (EPSRC) and will be running until March 2024. Supporting it will also be ADEPT, Highways England, Transport for London, Nottinghamshire County Council, Cooper Technology and the Road Surface Treatment Association (RSTA).

GPS need training to tackle chronic opioid use

GPs must be better-equipped to support patients to manage the psychological challenge of reducing their opioid use - according to research from the University of East Anglia. The recommendation is part of a toolkit launched to help GPs reduce the amount of opioids they prescribe. The toolkit outlines seven areas of best practice to tackle chronic opioid use – based on international research evidence, the experiences of health organisations and individual practitioners. It comes after figures for England and Wales revealed an increase in opioid prescriptions of more than 60 per cent in a decade – from 14 million in 2008 to 23 million in 2018. Lead researcher Dr Debi Bhattacharya, from UEA’s School of Pharmacy, said: “Opioids, like morphine, tramadol and fentanyl, can be effective for the short-term management of severe pain. However, they are highly addictive which makes stopping difficult yet long-term use can impair quality of life and overuse can be deadly.” “GPs and other health professionals need to urgently, proactively work with patients prescribed long-term opioids for non-cancer pain to gradually reduce or ‘taper’ their doses. “But if GPs are expected to initiate discussions about tapering or stopping opioids, they must be equipped with training to manage the psychological challenges experienced by patients when trying to reduce their opioid use.” “Without this training, prescribers are reticent to open 'a can of worms' that they know they don't have the skills to manage.” “For opioid tapering interventions to be effective, GPs need training in giving their patients the skills to manage any withdrawal effects.” Dr Bhattacharya said: “There needs to be a clear expectation that opioid de-prescribing is the responsibility of the prescriber. “Incentives may help GPs and other health professionals to prioritise reducing the amount of opioids being prescribed to patients, particularly among those who have been taking them long-term. “Prescribers need to better understand the consequences of excess opioid use, and they need better guidelines about how to gradually reduce or ‘taper’ doses.” The ‘Toolkit for Tackling Chronic Opioid Use in Non-Cancer Pain’ has been supported by NIHR Collaboration for Leadership in Applied Health Research and Care East of England, hosted by Cambridgeshire and Peterborough NHS Foundation Trust.

Sociologist and UConn expert Dr. Matthew Hughey discussed the findings with the Washington Post: "A new experiment at Texas A&M University helps illustrate the surprising pattern, which has not been widely studied but tends to line up with Labor Department data reviewed by The Washington Post: Black workers are typically less likely to be hired than White workers with the same experience, but the gap closes in middle age. When he saw the chart above, University of Connecticut sociologist Matthew Hughey was struck by the steadiness of the trend for Whites, compared to the volatile swoop of the line representing Black workers. It shows hiring managers tend to accept White applicants at face value while subconsciously scrutinizing Black ones, he said. “Black people have always been more objectified, scrutinized and surveilled than White people,” Hughey said. “Every little thing is nitpicked on a résumé or explained as a possible red flag.” The larger pattern is common in government data, but the chart comes from a new analysis in the Journal of Policy and Management from Texas A&M economist Joanna Lahey, a widely cited authority on discrimination in the labor market. Lahey noticed the counterintuitive pattern of age discrimination against Black workers when she and her collaborator, Douglas Oxley, asked about 150 business and MBA students to evaluate about 40 résumés each. About a quarter of the students had previously screened résumés in the real world, and 11 percent had experience in human resources." If just the top five percent of carbon-emitting plants in the U.S. reduced emissions to the average intensity of all plants, overall emissions from the electricity sector would fall 22 percent. A new book co-authored by Wesley Longhofer, associate professor of organization and management at Goizueta Business School, offers new insights into a persistent problem—how to curb carbon emissions from top-polluting power plants around the world. In Super Polluters: Tackling the World’s Largest Sites of Climate-Disrupting Emissions (Columbia University Press), Longhofer and co-authors Don Grant and Andrew Jorgenson argue that reducing pollution from fossil-fueled power plants should start with the dirtiest producers. From data they gathered over eight years on the carbon emissions of every power plant in the world, they found that a small number of plants contribute the lion’s share of pollution. For instance, if just the top five percent of carbon-emitting plants in the U.S. reduced emissions to the average intensity of all plants, overall emissions from the electricity sector would fall 22 percent. The book also questions claims that improvements in technical efficiency will always reduce greenhouse gases. “It’s the paradox of efficiency,” Longhofer says. “Just because a plant produces power more efficiently doesn’t mean they’ll pollute less. It just becomes cheaper to produce.” As sociologists, the authors are the first to put the problem into context, investigating global, organizational, and political conditions that explain super-polluter behavior. They demonstrate energy and climate policies most effective at curbing power-plant pollution and show how mobilized citizen activism shapes those outcomes. “Climate change is fundamentally an organizational problem. Even if you think about the Paris Accords, it’s the power plants and the cars within those states that produce the emissions, not the states themselves,” Longhofer says. “What do we do with what we already know? Do Wholesalers Discriminate Against AI in Procurement Practices?

If we deploy automation without thinking strategically about intelligence, too, isn’t AI likely to backfire on us? Airplane manufacturer, Boeing, made headlines in 2019 for all the wrong reasons. Its 737 Max aircraft was indefinitely grounded after two fatal crashes in the space of just six months had claimed the lives of 346 people. Investigation into the accidents revealed that updates to an automated system – the Maneuvering Characteristics Augmentation System, known as MACS – had failed to integrate one of two intelligent sensors, meaning the system lacked a critical security backstop. As the aircrafts switched into autopilot mode shortly after takeoff, the error sent them both into fatal nosedives within minutes. These tragedies highlight an issue with automation that needs more focused attention says Goizueta’s Ruomeng Cui, assistant professor of information systems and operation management. And it’s this: if we deploy automation without thinking strategically about intelligence too, is AI likely to backfire on us? Cui is an expert in operations strategies in digital retail and platform markets. To better understand the challenges surrounding automation and intelligence in operational processes, she teamed with Shichen Zhang of Tianjin University and Rutgers’ Meng Li to explore how AI brings value in the procurement space. With Deloitte reporting that almost 45% of Chief Procurement Officers globally are now using, piloting, or planning to integrate AI into their operations, these insights should provide interesting food for thought, says Cui. “AI isn’t just about being quicker, it’s also about being smarter. It can deliver automation but can also deliver predictive intelligence; and while these two dimensions might be correlated, one doesn’t necessarily imply the other – as the Boeing example demonstrates,” says Cui. From the tech perspective, there’s a lot of buzz about how AI is helping to drive decision-making, she adds. But there is still plenty that we don’t know about the operational dimensions to using artificial intelligence. “With international procurement, you’re basically talking about big retailers going in and requesting prices for goods or products from wholesale suppliers. And that’s a process that could, in theory, lend itself very well to AI, since it can automate simple (and tedious) tasks over and over again. So there’s a significant potential gain in companies outsourcing this kind of task to the machine.” But although the potential might be clear, Cui and her colleagues believe that simply automating these processes might not in fact yield optimal results; and could in fact work against buyers by encouraging suppliers to quote higher prices than they might in personalized, human transactions. Humans or AI Chat Bots? “We speculated about the possibility of wholesalers discriminating against the AI,” says Cui. “Specifically, we wanted to know if the sellers would quote higher prices to AI bots than they would to human buyers, because at the end of the day these bots are just machines; they don’t bring the authenticity or sincerity of human beings.” When Machines are Smart, Discounts Rise “When wholesalers are just asked over and over for their prices, they know that they are dealing with a machine and the intuition is that the machine is not intelligent, that it doesn’t have market expertise, and that it isn’t capable of decision-making. There’s no incentive to build relationships or to engage in any kind of negotiating dynamic here.” The topic is fascinating, and given the increase of AI in the workplace – a timely one.
