Research team aims to enhance security of medical devices

Tamer Nadeem, Ph.D., the principal investigator of the VCU-based MedKnights project, explained that the project’s focus is on the Internet of Medical Things (IoMT). Nadeem and co-PI Irfan Ahmed, Ph.D., both associate professors in the VCU College of Engineering Department of Computer Science, recently received $600,000 from the NSF’s Office of Advanced Cyberinfrastructure to put together a framework to improve IoMT security. IoMT devices are used in a range of diagnostic, monitoring and therapeutic applications. IoMT includes patient monitors, ventilators, MRI machines — even “smart beds.” Ahmed cited the internet-connected insulin pump is a good example of an IoMT device. Internet connectivity allows for both monitoring and adjusting the dosage remotely — functions that require a high degree of security for patient privacy as well as safety. All IoMT devices are potentially vulnerable to ransomware, denial of service and other malicious hacker attacks. Nadeem points out that IoMT devices have a higher security requirement than traditional IoT devices such as smart doorbells and smart thermostats in homes. “The most important thing in the medical domain is privacy,” Nadeem said. “For IoT devices in your home, you wouldn’t care that much about privacy, but for medical devices, it is an essential thing. You wouldn’t want anyone to know what your health conditions are, or what problems you might have had.” The work of the MedKnights group is important, as the IoMT domain is expanding; there is growth in terms of types of devices, number of patients using them and number of IoMT vendors. Nadeem added that the COVID pandemic and accompanying quarantine and stay-home orders increased the focus of medical-technology providers on the possibilities of IoMT. “Talking to some of the medical-device providers, I’ve learned that they are considering a line of products where they can remotely monitor patients on those devices, and they also can configure those devices remotely,” Nadeem said. Security is a large concern for the new generation of devices, because the current IoMT devices have been hit hard by hackers, he said. Security is an issue that extends from the individual patient to the institution. “Statistics show there are a lot of ransom attacks being done on the health sectors during the pandemic,” Nadeem said. “That motivated us.” The MedKnights team’s preparation for taking on the dragon of malicious IoMT attacks includes building a “test bed,” an isolated hardware/software assembly that Nadeem says will mimic the internet-enabled hospital setting. “In the hospital environment, there’s set of rooms. Each room has a lot of medical devices; they could be wired, or they could be wireless devices,” he said. “But there is no way that we can do what we want to do in a hospital.” The test bed will incorporate IoMT datasets based on typical device behavior, traffic and known malicious attacks. Nadeem explained that MedKnights will explore vulnerabilities of various IoMT hardware and software by subjecting the elements of the IoMT test bed to a range of attacks. “We will try to see in real time how efficient our technologies to monitor or detect these attacks, then try to intervene if we notice any change in the activities on the network,” he said. “Now, if the attacks manage to get into the device, we would like to also to start to see whether we can monitor these devices and observe abnormality or any misbehavior.” Nadeem said the next step is to isolate the source of fishy activity in the test bed network and begin to reverse-engineer the malware. He explained the group will work on understanding the question by looking for the “hole” that created the vulnerability. Ahmed said the MedKnights will bring undergraduates into the project through DURI, the Dean’s Undergraduate Research Initiative at the VCU College of Engineering. High school students will have an opportunity to join the team through a similar program known as the Dean’s Early Research Initiative, or DERI. DURI and DERI are just two ways of getting younger scientists and engineers involved in actual research. “For the last couple of years, I’ve been contacted by local high schools to host a couple of their students during the summer,” Nadeem added. “The students were really excited about it. We came up with some nice ideas about how to extend that work to their classrooms. As we continue this project, we will reach out to the schools, because we would love having a couple of their students involved.”

“Financial technologies offer great promise to tackle climate change and provide pathways for developing sustainable economies and lifestyles,” says Aparna Gupta, a professor of quantitative finance at Rensselaer Polytechnic Institute and co-director of the Center for Research toward Advancing Financial Technologies (CRAFT), the first-ever fintech research center backed by the National Science Foundation. CRAFT brings together industry partners and policy makers to conduct research that is relevant for industry and has potential for commercialization. Dr. Gupta says that blockchain technologies combined with smart contracts and Internet of Things (IoT) devices are set to transform property and casualty insurance that is subject to increasing threats from climate change. Similarly, distributed ledger technologies can be utilized for issuing innovative climate finance securities, such as green bonds and climate derivatives, by facilitating traceable, transparent, and standardized transactions. Regulatory readiness to support blockchain-enabled green bonds and other climate finance securities issuance is underway across the globe. Climate fintech is also set to play a pivotal role in increasing renewable power generation and accelerating the transition to clean energy, according to Dr. Gupta. Digital lending platforms use crowdsourcing models to provide debt financing for residential solar energy systems. Climate-conscious consumers can make spending decisions that minimize their carbon footprint through solutions such as using a credit card that allows them to round up their purchases and use the change for planting trees. In the investment management and advisory space, there is a growing recognition of the need for environmentally sustainable investing. Responding to this need, fintech startups are offering platforms for clean energy investments and enabling investors to construct low-carbon-impact financial portfolios. “Financial technologies innovations are poised to transform almost all aspects of financial services, and in doing so, offer great opportunities to address climate change challenges,” Dr. Gupta says. In addition to her leadership in fintech, Dr. Gupta is at the helm of a team of financial and renewable energy experts developing risk management tools to incorporate renewable energy into the energy market. They will set and standardize risk factors to make it easier for this critical industry to be both productive for investors and creators and systematized for users, similar to the rating system created for the bond market. One in seven Americans suffers from fecal incontinence, but is anyone talking about it? Augusta University expert offers treatments, research to help

Bowel or fecal incontinence, according to the Mayo Clinic, “is the inability to control bowel movements, causing stool (feces) to leak unexpectedly from the rectum. Also called bowel incontinence, fecal incontinence ranges from an occasional leakage of stool while passing gas to a complete loss of bowel control. Common causes of fecal incontinence include diarrhea, constipation, and muscle or nerve damage. The muscle or nerve damage may be associated with aging or with giving birth.” Dr. Satish Rao is a seasoned gastroenterologist and an expert in digestive health, particularly the brain-gut connection. Rao, a professor of medicine at the Medical College of Georgia at Augusta University, recently offered a Q&A on the topic of fecal incontinence with the journal Gastroenterology & Hepatology. What is the prevalence of fecal incontinence in the adult population? Surveys have indicated a prevalence of approximately 9% to 10% in the United States. A recent study reported a 14% prevalence, although this study was Internet-based and, thus, may not have included many elderly patients, as they may not be as computer-savvy as younger patients. It is safe to say that one in seven Americans currently suffers from fecal incontinence. Prevalence appears to be equal in men and women, although women outnumber men almost three to one when it comes to gastroenterology clinic visits and health care-seeking. Men may be too embarrassed to bring the issue of fecal incontinence to the attention of a physician, but when asked about it, they will admit and discuss it. Also, extracting information from a patient about fecal incontinence depends on how the question is asked. Asking patients whether they have daily leakage vs whether they ever have had leakage or have had leakage in the past month will elicit different responses that a clinician may interpret differently. It is important to remember that leakage is not a physiologic event that a healthy adult should have at any time, even once a month or once a year. Not having the capacity to control bowel evacuation or having leakage unaware of its occurrence signals an abnormality. What are notable risk factors for fecal incontinence? In women, pregnancy can be a risk factor, particularly if giving birth involves pelvic tissue damage, such as injury inflicted by forceps use or the unfortunate occurrence of a significant tear. Neurologic or back injuries are other common risk factors. Also, chronic diarrhea can progress to fecal incontinence owing to severe irritation of the rectum or irritants in stool. Further, any condition that changes the ability of rectal capacity can result in fecal incontinence. These circumstances can include surgery or radiation to the rectal area. Hear from a patient and learn more about Rao's research using magnetic stimulation to treat fecal incontinence. What treatment modalities are currently available? Simple, conservative treatment consists of educating patients about fecal incontinence and instructing them to avoid precipitating events. For example, although many people love to have a meal followed by a cup of coffee and a walk, such a sequence of activities is ill-advised for an incontinent patient: the meal provokes a gastric-colonic response, coffee is a powerful colonic stimulant, and exercise also stimulates motility. This triad creates the perfect storm for a stool leakage or accident while the patient is out on the after-dinner walk. Antidiarrheal therapies can be very effective but only in approximately 15% to 20% of patients. Another treatment is biofeedback, which can correct muscle weakness using behavioral techniques. Biofeedback provides resolution in approximately 50% to 70% of patients. The traditional model of office-based biofeedback requires that the patient make 6 or even up to 10 visits to a specialty clinic. This may mean that some patients must drive very long distances to an appropriate care facility that is staffed with trained personnel or physical therapists. This scenario presents a significant challenge for many patients, which is increasingly being recognized by health care professionals and researchers. Good devices for home-based biofeedback have been scarce; however, such a device was recently approved by the US Food and Drug Administration. The research center at Augusta University has tested it in a clinical trial setting and found it to be quite effective as a home biofeedback treatment. Dextranomer is another treatment modality. It involves injection of small beads of dextran polymers into the anorectal region. The beads form a protective cuff or a buffer to stop stool leakage. Another treatment modality is sacral nerve stimulation using the Medtronic InterStim system. The patient is outfitted with a pacemaker-like device with wires that continuously stimulate the sacral nerves that control stool events. In the case of a torn muscle, suturing the torn ends to reduce the size of the anorectal opening is usually useful for women postpartum, although the effect may not be sustained in the long term. What emerging treatments and research should clinicians be aware of? One emerging treatment developed at Augusta University’s Clinical Research Center is called translumbosacral neuromodulation therapy (TNT). TNT is similar to TAMS and involves the fecal delivery of magnetic energy through an insulated coil to the lumbosacral nerves that regulate anorectal function. The pulses generated are of the same strength as those of magnetic resonance imaging. The team at Augusta University’s research center has shown that TNT mechanistically improves nerve function and substantively improves stool leakage. A sham-controlled study and long-term study are currently underway at Augusta University and Harvard University’s Massachusetts General Hospital. These studies are being sponsored by the National Institute of Diabetes and Digestive and Kidney Diseases. A multicenter study sponsored by the National Institutes of Health that the team at Augusta University also is involved with is the FIT (Fecal Incontinence Treatment) trial. This randomized study compares biofeedback with dextranomer injection. Also, as mentioned, tools are becoming available for home biofeedback that should allow many more affected patients to receive treatment because they can do so in the comfort of their own home. The research center at Augusta University is working on a novel home biofeedback protocol for the treatment of constipation and fecal incontinence. Kelley expert: Olympics could be targeted by hacktivists, others seeking to embarrass Beijing

Ask an Expert: Are airline concerns over 5G technology warranted?

Airlines around the world announced they were grounding planes this week as the rollout of 5G technology was set to begin. Major providers AT&T and Verizon halted their 5G rollout plans amidst the airlines’ claims that the technology would render a large swath of their aircraft unusable. Dr. Steven Holland, an associate professor at Milwaukee School of Engineering, helps sort out the validity of the airlines’ claims and explain why they are so concerned. Dr. Holland is an expert in electromagnetics, antennas and microwave/radio frequency circuits. “5G wireless systems promise transformational increases in wireless internet speed and connectivity. A key to the higher network speeds is increased 5G radio frequency spectrum usage. While reduced speed 5G has been operational in parts of the US for several years, new 5G frequency spectrum was set to become operational in early December 2021 and offer significantly higher network speeds. “However, rollout was delayed one month when the FAA raised warnings that 5G towers operating in this new spectrum could interfere with airplane radar altimeters, which operate at frequencies just above this new 5G spectrum. Altimeters are critical instruments in modern aircraft that measure a plane’s altitude, and thus potential interference could be disastrous. “While the altimeter and 5G frequency bands do not overlap, they are relatively close together, and the altimeter standards were set decades ago when the radio spectrum was far less crowded. As a result, the altimeter circuitry was not designed to operate in the vicinity of systems like the 5G towers that will transmit radio signals so close to the altimeter frequency range. “Due to limited testing, the FAA and FCC were not able to determine conclusively if the interference concern is warranted before the month-long delay expired in early January, and so an additional two-week delay was instituted. This brings us to today, January 19th – and still no conclusion has been reached by the FCC and FAA. To avoid further rollout delays, today several wireless carriers deployed the new 5G operating band on their networks except for those towers near airports. More extensive testing is needed to determine whether 5G interference is truly a risk to the altimeter systems.” Dr. Holland is available to speak with media about 5G technology and how it can impact aircraft.
